This session will introduce the Age Friendly Care at Home initiative that can help hospice and home health agencies elevate the care and services they provide. Participants will learn how to apply the Age Friendly Care Framework to achieve patient centered care, improve patient outcomes, improve staff job satisfaction and create a competitive edge for their agencies. - Examine the Age Friendly Care Approach – the 4M’s Framework
- Explore how the the 4M’s Framework with create a shift in care
- Analyze the key drivers of Age Friendly Care
- Put the 4M’s Framework into practice through case studies
- Clarify the benefits of Age Friendly Care for patients, staff and agency
- Review the participating Age Friendly Health Systems
